What is Helfo and how does it benefit international workers in Norway? Helfo (the Norwegian Health Economics Administration) is the governmental body responsible for managing health expense reimbursements, ensuring that all taxpayers have access to subsidized healthcare. Once an individual pays out-of-pocket medical expenses and hits the strict statutory maximum of 3,278 NOK within the 2026 calendar year, Helfo automatically issues a healthcare exemption card (frikort), waiving future approved medical costs. For cross-border employees living in the EU/EEA while working in Norway, Helfo processes the S1 Certificate to secure healthcare coverage in their home country.

To prevent gaps in medical coverage, these cross-border workers must be registered via the S1 Certificate, which transfers healthcare cost burdens between nations under EU social security coordination rules.

The Procedural S1 Application Timeline

  1. Submit the S1 Application: The employee (or the employer acting with an authorized power of attorney/fullmakt) must submit the S1 application to Helfo digitally through the Altinn portal or via physical mail. The application must strictly include a copy of the current Norwegian employment contract. If the contract is over 12 months old, a signed employer confirmation proving active employment is mandatory.
  2. Helfo Processing and Approval: Helfo audits the application to verify valid Folketrygden membership. The standard administrative processing time for an S1 Certificate is up to 4 weeks. Once approved, Helfo issues the formal S1 document directly to the user digital mailbox or their physical postal address. 
  3. Home Country Registration: The worker must physically or digitally deliver the issued S1 Certificate to the public health insurance authority in their home country of residence.
  4. Finalization of Coverage: The health authority in the country of residence sends an electronic confirmation back to Helfo to finalize the cross-border registration. The employee and their dependent family members are then fully entitled to state-funded medical treatment in both their country of residence and Norway.
